Issue of Identity Cards.

Application for a new Identity Card (New Identity Card - DPR 1, Obtaining a duplicate - DPR 7, Amending the NIC - DPR 8)
A person aged 15 years is subjected to registration of the Identity Card.
Original Birth Certificate or Probable Age Certificate and a photocopy for the applicants less than 50
For over age 50, If there isn’t the original birth certificate or probable age certificate, there should be document search result chart and optional documents (certified certificate of school leaving, bridging certificate, Birth certificate of a child that applicant’s birth date is mentioned, birth certificate note, estate leaving certificate, certified copies of the passport , Registration card issued by the estate superintendent, birth certificate)
Citizenship certificates if born abroad
Iconic standard four passport size colour photographs
If you are applying for an NIC for the first time, the certified photocopy of the National Identity Card of the applicant's parent / guardian which is certified by Grama Niladhari
Marriage certificate to get husband's name
Certificate of Samanera or an upasampada certificate for Buddhist monks
Disrobed certificate issued by the Department of Buddhist Affairs, if the Buddhist monk is disrobed.
Certificate of Service obtained within 3 months if required
For missing identity cards, the original copy of the complaint made by the police
Identity card currently being used for amendment
Formal affidavit with stamp (if needed)
Revision of fees for obtaining an Identity Card from 2024
Obtaining the Identity Card for the first time Rs. 320.00
Obtaining the amendment identity card Rs. 620.00
Obtaining duplicates for missing identity cards Rs. 1200.00
A first-time applicant over the age of 15 will be subjected to fine of Rs. 2500.00 in addition to the above Rs.320.00

Issuing Birth, Death and Marriage Certificates

    Application can be obtained from the District Registrar's Division at any Divisional Secretariat.
    Charges
        If you know the number of the birth certificate and the registered date the fee is Rs. 100.00 per copy
        If you do not know the birth certificate number and the registered date, the fee for one copy is Rs.
    Make necessary payments to Divisional Secretariat.
    Duly filled application should be submit to the relevant Divisional Secretariat.
    If you like to obtain the birth certificate by post, submit a stamped envelop with the application.
    If the birth is registered, a birth certificate is sent and if the birth is not registered a letter will be sent.

Photocopies of Mother and Father National Identity Cards
If the birth is less than 1 year, stamps of Rs. 50
100 stamps if more than 1 year

Please bring the above documents to the District Registrar of the Department with Parent and child.

Registration of a Past Death

If a death has not been registered with 03 months since its occurrence it may be registered.
The death can only be registered only if a period of 25 yrs. has not been lapsed since occurrence of such death
Persons eligible for this,
- Any party having interest of
- One of the parents
Declaration can be produced by,
- Close relation present at the time of death or who took care of the deceased when he/she was ill for the last time
- Any other party interested
Fee chargeable is Rs.50.00
